Ms. Carolyn White admitted to Christian Care Center of Medina following a hospital stay as a result of a motor vehicle accident. Upon evaluation, she was observed to have a decline in self-care participation, balance, safety and strength requiring skilled Occupational, Physical and Speech therapy services.
Ms. White participated in a challenging Therapy program in order to increase her ability to return home independently. As a result of her efforts, she is returning home independent with transfers, self-care tasks and walking more than 500 feet.

Mr. Danny Stewart admitted to Christian Care Center of Medina following a hospital stay as a result of a hip fracture. Danny was referred to skilled Physical and Occupational therapy for intensive rehabilitation in order to return home independently. Upon evaluation, he presented with decreased safety and independence with functional mobility, impaired strength, and reduced activity of daily living / instrumental activity of daily living participation.
Our Skilled Rehab Team developed a custom treatment plan targeting Danny’s impairment in order to address his deficits and return home with independence. As a result of Danny’s commitment to his plan of care, he is returning home independent with mobility utilizing a front wheeled walker, independent with ascending/descending stair casings, and independent with all self-care tasks.
We have loved having Danny and wish him the best of luck as he transitions home.
Ms. Judy Hample admitted to Christian Care Center of Medina from an outside facility due to a decrease in ability to care for herself independently. Upon evaluation, she was observed to have decreased balance, safety insight, coordination, strength and self-care independence. Ms. Hample received skilled Occupational, Physical and Speech Therapy in order to increase her overall function to return home with increased independence and safety.
Following a rigorous Therapy program, Ms. Hample is now able to perform self-care tasks with modified independence, walk more than 300 feet, complete transfers independently and perform tasks of increased complexity with safe decision-making skills.

Ms. Glenda Kennedy admitted to Christian Care Center of Medina for further rehabilitation following a right distal femoral fracture resulting in an open reduction and internal fixation (ORIF) being performed. She was referred to Physical and Occupational Therapy services due to a decline in activities of daily living and mobility since recent fall and post-surgery.
Ms. Kennedy participated in an individualized treatment plan tailored to her specific needs and goal aspirations in order to return home with limited to no caregiver assistance. As a result of her hard work and dedication, She is able to return home independently with mobility utilizing a front wheeled walker, independent with ascending/descending stair casings, and independent with all self-care tasks.
When asked about her therapy experience at Christian Care Center of Medina, she states, “The therapy team has not only gotten me back to where I was before my surgery…. But better!”
We have loved having Ms. Kennedy and wish her the best of luck as she transitions home and into outpatient therapy!

Amanda has worked in long-term healthcare for over 20 years and 9 months at Christian Care Center of Medina. She continually shows a “Commitment to Caring” when it comes to the Residents, family members and staff. Her motivation is receiving a renewed sense of job satisfaction every day as she continues to provide caring and compassionate service.
She shows her “Commitment to Caring” for the Residents by always interacting and caring for them with respect and kindness. Integrity is also extremely important to her as she keeps her word even when it is difficult. Her compassion gives her the ability to bond and care for others.
Amanda likes working at Christian Care Center of Medina because of it’s well-rounded and supportive environment that has shown continued success.
One of her best personal experiences of working is listening to her Resident’s stories and experiences
and enjoying their sense of humor.
Mr. Thomas Bray admitted to Christian Care Center of Medina following a hospital stay as a result of new onset of generalized weakness, COPD exacerbation and pneumonia. Upon evaluation, he was observed to have increased shortness of breath with activity, decreased activity tolerance, strength and balance.
He received skilled Occupational, Physical and Speech therapy during his stay in order to regain his independence to return home. Within just a few short weeks, Mr. Bray is able to return home independent with self-cares, mobility and transfers.
We are extremely proud of Mr. Bray and all his success! When asked about his Therapy experience at Christian Care Center of Medina, he states, “You all have been so great. I highly recommend the therapy department here in Medina!”

Ms. Maggie Brasfield admitted to Christian Care Center of Medina following a hospital stay as a result of a fall. She participated in skilled Occupational, Physical and Speech therapy services during her stay. Upon evaluation, she presented with decreased balance, coordination, safety awareness, strength, mobility and self-help skills.
Ms. Brasfield worked exceedingly hard to challenge herself outside of her comfort zone in order to meet her goals. As a result of this, she is returning to the local Assisted Living Facility independent with mobility and self-care tasks.
When asked about her therapy experience at Christian Care Center of Medina, she states, “Everybody was so nice. You all did so much to help me. Stephen and I are very grateful.”
Mr. Gordon Fields admitted to Christian Care Center of Medina following a hospital stay as a result of increased episodes of shortness of breath with activity. Upon evaluation, Mr. Fields presented with impaired balance, coordination, strength and problem solving resulting in limitations and participation in areas of self-care and mobility.
After participation in a rigorous Occupational, Physical and Speech therapy program his ability to participate and complete day-to-day activities with modified independence improved tremendously. Mr. Fields is now able to return home with his wife and complete daily tasks with improved independence.
We are extremely proud of Mr. Fields and his determination to return home to be with his wife. When asked about his therapy experience at Christian Care Center of Medina, he states, “I have gained so much… I am much better than I was before. When I got here, I felt like a burden to my wife but because of the therapy team here I feel confident returning home!”
Oh… and GO VOLS!
Mr. James Spencer admitted to Christian Care Center of Medina following a hospital stay as a result of a sudden change in medical status. After thorough evaluation, Mr. Spencer was found to have increased weakness, decreased balance, increased need for assistance with self-care tasks and difficulty ascending/descending stair casings. He received skilled Occupational and Physical Therapy to assist him in re-gaining his independence in functional mobility and self-care tasks in order to return to his prior level of function.
As a result of Mr. Spencer’s efforts to achieve his goals, he is discharging Christian Care Center of Medina completely independent in all self-care tasks, ascending/descending multiple level stair casings and ambulating over 300 ft.
When asked about his therapy experience at Christian Care Center of Medina, he states, “The therapy team has been better than good… they have been great! My progress got better every single day. They helped me build my strength back. I can’t say thank you enough!”

Mr. Charles Bradley admitted to Christian Care Center of Medina following a hospital stay as a result of a fall. Upon evaluation, Mr. Bradley was observed to have a functional decline in self-help skills, strength, coordination and balance making it difficult for him to enjoy leisure and other meaningful tasks.
After thorough participation in skilled Occupational, Physical and Speech Therapy, his ability to engage in day-to-day tasks with improved balance, strength and safety awareness improved remarkably. He is returning home with his wife completely independent in mobility, transfers and self-care tasks.
We are so proud of Mr. Bradley and all his hard work towards achieving his goals! When asked about his therapy experience at Christian Care Center of Medina, he states, “Kohnley (PTA) and Brooklyn (COTA/L, RD) have done so much for me to get me where I am today. They are truly the best!”
Mr. John Crosser admitted to Christian Care Center of Medina due to a decrease in strength, functional mobility, coordination and increase in need for assistance with personal care tasks. Due to the recent onset of symptoms, he began a demanding rehabilitation journey through Physical, Occupational and Speech Therapies and was able to complete the rehab to home journey within two weeks. When Mr. Crosser admitted, he required moderate physical assistance for performing day-to-day tasks. As a result of Mr. Crosser’s goal-minded mindset and determination, he is now able to return home with his wife independently.
When asked about his Therapy experience at Christian Care Center of Medina, he states, “My experience here was great! The Therapy department was very nice!” Congratulations on your success and we wish you the best!
What is Physical Therapy?
Physical Therapy in a Skilled Nursing Facility plays an essential part in improving a resident’s health and safety. Your Physical Therapist and/or Physical Therapy Assistant will focus on maintaining, promoting, and/or restoring lost physical function following a serious injury or illness. Physical Therapists in a SNF work side by side with the interdisciplinary team in order to design individualized therapy plans that address each resident’s unique needs. Interventions that may be deemed appropriate would include but are not limited to strength training, fall prevention and balance/gait training.
Here at Christian Care Center of Medina, the Physical Therapy team prides themselves on providing unique, individualized treatment approaches tailored to your personal goals. They are skilled in adapting the environment to promote successful rehabilitation in preparation for the transition from SNF to home.
Christian Care Center of Medina is hosting a Community Toy Drive and Resident Angel Tree!
For our Toy Drive, we are gladly accepting toy donations for the children of our community. Please have your donation dropped off at our Facility by Monday, December 5th. Gifts do not need to be wrapped.
As for our Resident Angel Tree, stop by our Facility and take a paper Angel with a Resident’s name on it from our Christmas Tree. On the back you will see some suggestions of items to purchase for that Resident. After picking an Angel, write your name, along with the Resident’s name, on the sheet next to the Christmas Tree. Be sure to wrap your gifts and return them back to the Facility and give them to Lee’ Anne Polasek or someone in the Front Office by Monday, December 12th. We would like to present the gifts to our Residents at the Facility Christmas Party on Thursday, December 15th.

The Residents at Christian Care of Medina celebrated our Veterans on Friday Nov. 11th. During the celebration, a special prayer was given by Kevin Autry, our Facility Chaplain, and Brad Rogers, our Executive Director, spoke. Thomas and Anita Darnell, representing the Gibson County American Legion Post #287, also gave special gifts to our Veterans.

Mr. Kenneth Frazee admitted to Christian Care Center of Medina following a CVA. He presented with weakness, poor coordination, difficulty performing meaningful tasks and impaired mobility. He required skilled therapy to be able to return home as quickly and independently as possible.
He received Occupational, Physical and Speech therapy services that aimed to achieve client-centered goals that were meaningful to him. As a result of his dedication, he was able to exceed his goals and return home! Now, he can safely walk with a rolling walker, perform his own activities of daily living and go up and down stairs independently.
When asked about his therapy experience at Christian Care Center of Medina, he states, “You all have done so much for me and I am so grateful!”
We are exceptionally proud of Mr. Frazee and wish him the best of luck as he transitions home!
Ms. Regina Gordon admitted to Christian Care Center of Medina following a hospital stay after a new onset of generalized weakness affecting her ability to complete meaningful activities. She required skilled therapy to be able to return home with as much strength, endurance, and balance as possible.
Ms. Gordon received Occupational, Physical and Speech therapy that focused on attainable ways to reach goals that were meaningful to her in order re-enter the community safely. As a result of her dedication and willingness to try new things, she was able to exceed her goals! Now, she can safely walk with a rolling walker, perform her own bathing/dressing tasks, and go up and down stairs independently.
When asked about her therapy experience at Christian Care Center of Medina she stated, “I’m this successful because I had the best therapists to get me there!”
We are so incredibly proud of Ms. Gordon’s accomplishments! We thank you for entrusting us to serve you throughout your rehabilitation journey.
Ms. Rutha Robertson admitted to Christian Care Center of Medina after experiencing a fall resulting in a left hip fracture. She presented with decreased ability to perform ambulation, transfers, and bathing/dressing tasks. Ms. Robertson received skilled Occupational, Physical and Speech therapy during her stay to maximize her rehab potential and assist her in meeting her goal to return home independently. While here, she participated in various treatment approaches to challenge her physically and mentally for the best possible outcome measures.
At time of discharge, Ms. Robertson achieved her goal of walking, tolerated standing for extended periods of time and progressed to being independent with all activities of daily living.
When asked about her therapy experience, Ms. Robertson stated, “I can’t say enough good things about this place, from the cleanliness, great food and smiling faces. The therapists went above and beyond, always going the extra mile. I feel confident returning home. If you ever have a problem, Medina is the place to go!”

Ms. Jeanette McAlister came to Christian Care Center of Medina due to an overall decline in functional status including decreased balance, strength, safety, and independence with self-help skills making it difficult to complete day-to-day tasks. Ms. McAlister received Occupational, Physical and Speech therapies which assisted her in developing strategies to regain her independence and return home! She participated in fun, therapeutic activities that addressed her areas of concern to successfully meet all her goals.
Pictured, you will see Ms. McAlister and our PTA, Kohnley, admiring and posing in front of the facilities Military Armed Forces Memorial. Ms. McAlister states, “This place is like a family… it’s like home.”
We will miss her as she transitions home but we are all so proud of the accomplishments she has made during her rehab stay.
