August 12, 2025

Ms. Ann Studards was referred to our outpatient Physical Therapy department from her Primary Care Physician due to increased weakness impacting her quality of life and independence at home. 


During her initial evaluation, Ms. Ann presented with difficulty in mobility, ascending/descending stairs and decreased confidence in balance, with a high fall risk. Our Outpatient Team developed a personalized treatment plan tailored to Ms. Martha’s challenges. Key interventions included gait training with a focus on endurance and safety, balance and proprioception exercises using dynamic surfaces, strengthening of the lower extremities and home safety education and caregiver involvement. 


After 4 weeks of therapy, Ms. Ann is graduating from our Outpatient Therapy program walking independently with a rolling walker, successfully navigating stairs without assistance, significant boost in confidence with reduced fear of falling and successfully returned to community activities. 


Ms. Ann’s Physical Therapy Assistant, Kohnley Sumner, states, "Her motivation and dedication were truly inspiring. She progressed from needing assistance with basic tasks to fully managing her day-to-day life. Our goal is always to restore independence and quality of life—and she achieved just that." 


At discharge we asked Ms. Ann about her therapy experience to which she stated, “You ladies in therapy are truly amazing! Even my daughters were impressed—and when you pass their inspection, you know you're doing something right. Your constant smiles and positive energy make it easy for me to show up, stay motivated, and enjoy working with such a wonderful team.”

Ms. Peggy Scroggins admitted to Christian Care Center of Medina following a hospital stay resulting in significant functional limitations in self-care, mobility and safety awareness. 


Upon admission, initial challenges included unsteady gait with frequent loss of balance, impaired insight to safety risks and caregiver dependency with self-cares. 


Our interdisciplinary team developed a focused treatment plan addressing her fall risk, strength and functional mobility which included balance training, activities of daily living retraining with focus on safe task sequencing, gait training with assistive devices, safety awareness education and cognitive support to improve hazard recognition and problem-solving. 


In less than 4 weeks, Ms. Peggy was able to safely discharge home independent with mobility and self-cares while also recognizing safety hazards at 100%. Our interdisciplinary team worked together to find and enroll her in home health therapy for continued progress. 


Ms. Peggy states, “When I came here the goal I set for myself was to get stronger and get back home. Now that I’ve achieved just that- I’m not so sure I want to leave this place. You all have turned into family, and I can’t think of anything more rewarding than that!” 


Ms. Peggy’s story is a shining example of how targeted, skilled therapy combined with patient motivation can overcome even the biggest obstacles. Her journey reminds us that independence is always worth fighting for—and achievable with the right support!

Mr. Samuel Bowman was a long-term care resident with us here at Christian Care Center of Medina. He was referred to our Rehabilitation Department to improve his balance and mobility. Mr. Bowman was eager to participate in therapy, demonstrating daily improvements. In less than three weeks, Mr. Bowman not only achieved his goals but surpassed them! 


Due to Mr. Bowman’s efforts with our Rehabilitation Team, he achieved his personal goal of residing at local Assisted Living/Independent Living facility. We asked Mr. Bowman about his therapy experience to which he responded, “Therapy was something I looked forward to. You all invested time in me and for that I am thankful. I give you 5 stars!”

Ms. Caraway admitted to Christian Care Center of Medina following a lengthy hospital stay that left her feeling weak, unsteady and fearful of what the future would hold. Upon evaluation by our skilled rehab team, deficits were noted in endurance, mobility and self-care performance. During Ms. Caraway’s evaluation, she feared she would never return to a local assisted living facility to reunite with her husband. With the help of our dedicated therapy staff, she eagerly began her inpatient rehabilitation program while pushing those fears aside. 

Ms. Caraway’s therapy program began with strength training, balance drills and activities of daily living to increase her level of independence in order to achieve her most desired goal of reuniting with her husband. 


Each week, Ms. Caraway grew stronger. Her posture and balance improved, her steps became more confident, and most importantly—so did her mindset. In just under 30 days, Ms. Caraway was achieving her goals and returning home to her husband. 


We asked Ms. Caraway about her therapy experience, to which she responded, “This place exceeded my expectations. I knew I would be in good hands, but you all blew it out of the water!” 


Ms. Caraway’s story is a great reminder that it’s never too late to rebuild strength and reclaim independence. We’re proud to be apart of her journey.

Mr. Gary Martin chose Christian Care Center of Medina after a recent decline in function due to a recent hospitalization. Upon admission, he required increased assistance with generalized mobility and activities of daily living, impacting his ability to return home with his wife.


Mr. Martin was an eager therapy patient! He participated in Physical, Occupational, and Speech Therapy with the determination to return home. In just a few weeks, he was able to achieve the ultimate goal of returning home! He stated, “When I arrived at Christian Care, I was completely immobile. These girls in therapy provided me with the encouragement and skills to do most everything independently. I want to thank the therapy team for always meeting me with kindness and compassion."

We are proud to announce the rehabilitation success of Mr. Hubert Cantrell! 


Mr. Cantrell admitted to Christian Care Center of Medina following a recent hospital stay. Upon admission, he presented with increased weakness and an overall functional decline limiting his ability to live independently. 


Mr. Cantrell worked daily in and out of the therapy gym to increase his strength, mobility and independence to achieve the ultimate goal of returning home. Within two weeks, he was able to exceed his goals and return home independently! 


We asked Mr. Cantrell about his stay with us, and he states, “If this team was on a list a mile long, I would have this group at number one!” 


It was a pleasure to serve Mr. Cantrell and his family. Please join us to congratulate Mr. Cantrell on his success!
