What is Physical Therapy?
Physical Therapy in a Skilled Nursing Facility plays an essential part in improving a resident’s health and safety. Your Physical Therapist and/or Physical Therapy Assistant will focus on maintaining, promoting, and/or restoring lost physical function following a serious injury or illness. Physical Therapists in a SNF work side by side with the interdisciplinary team in order to design individualized therapy plans that address each resident’s unique needs. Interventions that may be deemed appropriate would include but are not limited to strength training, fall prevention and balance/gait training.
Here at Christian Care Center of Medina, the Physical Therapy team prides themselves on providing unique, individualized treatment approaches tailored to your personal goals. They are skilled in adapting the environment to promote successful rehabilitation in preparation for the transition from SNF to home.
Christian Care Center of Medina is hosting a Community Toy Drive and Resident Angel Tree!
For our Toy Drive, we are gladly accepting toy donations for the children of our community. Please have your donation dropped off at our Facility by Monday, December 5th. Gifts do not need to be wrapped.
As for our Resident Angel Tree, stop by our Facility and take a paper Angel with a Resident’s name on it from our Christmas Tree. On the back you will see some suggestions of items to purchase for that Resident. After picking an Angel, write your name, along with the Resident’s name, on the sheet next to the Christmas Tree. Be sure to wrap your gifts and return them back to the Facility and give them to Lee’ Anne Polasek or someone in the Front Office by Monday, December 12th. We would like to present the gifts to our Residents at the Facility Christmas Party on Thursday, December 15th.

The Residents at Christian Care of Medina celebrated our Veterans on Friday Nov. 11th. During the celebration, a special prayer was given by Kevin Autry, our Facility Chaplain, and Brad Rogers, our Executive Director, spoke. Thomas and Anita Darnell, representing the Gibson County American Legion Post #287, also gave special gifts to our Veterans.

Mr. Kenneth Frazee admitted to Christian Care Center of Medina following a CVA. He presented with weakness, poor coordination, difficulty performing meaningful tasks and impaired mobility. He required skilled therapy to be able to return home as quickly and independently as possible.
He received Occupational, Physical and Speech therapy services that aimed to achieve client-centered goals that were meaningful to him. As a result of his dedication, he was able to exceed his goals and return home! Now, he can safely walk with a rolling walker, perform his own activities of daily living and go up and down stairs independently.
When asked about his therapy experience at Christian Care Center of Medina, he states, “You all have done so much for me and I am so grateful!”
We are exceptionally proud of Mr. Frazee and wish him the best of luck as he transitions home!
Ms. Regina Gordon admitted to Christian Care Center of Medina following a hospital stay after a new onset of generalized weakness affecting her ability to complete meaningful activities. She required skilled therapy to be able to return home with as much strength, endurance, and balance as possible.
Ms. Gordon received Occupational, Physical and Speech therapy that focused on attainable ways to reach goals that were meaningful to her in order re-enter the community safely. As a result of her dedication and willingness to try new things, she was able to exceed her goals! Now, she can safely walk with a rolling walker, perform her own bathing/dressing tasks, and go up and down stairs independently.
When asked about her therapy experience at Christian Care Center of Medina she stated, “I’m this successful because I had the best therapists to get me there!”
We are so incredibly proud of Ms. Gordon’s accomplishments! We thank you for entrusting us to serve you throughout your rehabilitation journey.
Ms. Rutha Robertson admitted to Christian Care Center of Medina after experiencing a fall resulting in a left hip fracture. She presented with decreased ability to perform ambulation, transfers, and bathing/dressing tasks. Ms. Robertson received skilled Occupational, Physical and Speech therapy during her stay to maximize her rehab potential and assist her in meeting her goal to return home independently. While here, she participated in various treatment approaches to challenge her physically and mentally for the best possible outcome measures.
At time of discharge, Ms. Robertson achieved her goal of walking, tolerated standing for extended periods of time and progressed to being independent with all activities of daily living.
When asked about her therapy experience, Ms. Robertson stated, “I can’t say enough good things about this place, from the cleanliness, great food and smiling faces. The therapists went above and beyond, always going the extra mile. I feel confident returning home. If you ever have a problem, Medina is the place to go!”

Ms. Jeanette McAlister came to Christian Care Center of Medina due to an overall decline in functional status including decreased balance, strength, safety, and independence with self-help skills making it difficult to complete day-to-day tasks. Ms. McAlister received Occupational, Physical and Speech therapies which assisted her in developing strategies to regain her independence and return home! She participated in fun, therapeutic activities that addressed her areas of concern to successfully meet all her goals.
Pictured, you will see Ms. McAlister and our PTA, Kohnley, admiring and posing in front of the facilities Military Armed Forces Memorial. Ms. McAlister states, “This place is like a family… it’s like home.”
We will miss her as she transitions home but we are all so proud of the accomplishments she has made during her rehab stay.
Some of our residents had the opportunity to visit the Pumpkin Patch last week. They really enjoyed the outing.
Ms. Clara Potts was referred to Christian Care Center of Medina due to new onset of decrease in strength, functional mobility, reduced dynamic balance and reduced daily activity participation.
She received Occupational, Physical and Speech therapies during her stay. Each skilled therapy incorporated various treatment approaches to reach goals that were meaningful to Ms. Potts. She enjoyed challenging herself and never said “no” to out of the box interventions. She was eager to put in the work and time, because of this, results were shown in record-time! Within a few short weeks she was able to meet all her goals and return home. We are so proud of the work she put in to achieving her goals!
When asked about her time spent at Christian Care Center of Medina, Ms. Potts states, “Everyone has been so good to me… Thank you to the staff for everything! I am grateful.”
Ms. Thelma White came to Christian Care Center of Medina with difficulty standing, walking and performing self-care tasks independently. She received Physical, Occupational and Speech therapies during her stay with each discipline developing a plan of care specific to her limitations. She demonstrated admirable motivation throughout her stay, because of this, she was able to meet her goals and return home.
Ms. Thelma was a joy to work with and always kept everyone around her laughing and smiling! We wish her the best as she transitions home. Thank you for choosing Christian Care Center of Medina for all your Rehabilitation needs!
Mr. Don Lannom came to Christian Care Center of Medina with increased weakness, impaired strength and decreased ability to perform meaningful activities such as walking, bathing and dressing due to a surgical wound on the left heel.
He received skilled Physical and Occupational Therapy where the Rehab Team worked hard to develop patient-driven interventions to meet his goals to return home and resume working despite being toe-touch weight bearing on the left leg. As a result of his dedication and perseverance, he overcame multiple obstacles and with the appropriate modifications recommended by our skilled therapists he is returning home!
Mr. Lannom states, “These rehab girls are the real deal! They have helped me become so much stronger than I was before my injury!”
The following is from a family member of one of our residents who passed away last night. It was sent to our Executive Director, Brad Rogers.
Saw A Nurse Tonight!
I saw a nurse tonight as she limped to her car, blisters on her feet where she ran from patient to patient tending to their needs. Her tired body dragging just to get to her car to sit down. I can't even begin to tell you how many steps she took. I saw her put her patients’ needs above hers!
I watched her as she fed a patient, that no one else could get to eat. I watched her as she held the hand of a patient taking their last breath. I saw her cry with a grieving family! I saw her stay several hours after her shift ended to comfort, hug and hold a hand!
I saw a nurse tonight put someone else's family before her own. Not because she had to, but because she's a nurse and that's what she does!
Amanda Hoffman (Nurse at Christian Care Center of Medina) you will forever be engraved in my heart! I saw the true definition of what a real nurse looks like! We have a bond that will never be broken.

There is a chill in the air and it’s beginning to look like Fall at Christian Care Center of Medina. Our MDS Coordinator Niki Upton’s sons, Knox Sanford (4 years old) and Andrew Upton (8 years old), grew and donated these wonderful pumpkins and gourds to our Facility, which they call “momma’s nursing home.”
Christian Care Center of Medina is pleased to announce the addition of Kristen Hunt, an affiliate of Comprehensive Rehab Consultants! Kristen is a physiatrist who will be working closely with our Rehab Department and Physicians in order to improve patient progression by adding in a layer of clinical oversight and capabilities that do not currently exist in most skilled nursing facilities.
You may be wondering, What is a Physiatrist? or Why does my loved one need this service?
Physiatrists are trained to focus on the whole patient and how each of the body’s major organ systems impact function. Kristen will work side-by-side with our skilled therapy staff to aid in patients achieving maximum function and the highest quality of life possible. The practice of physiatry is built on an interdisciplinary team approach that addresses a patient’s physical, emotional, and psychosocial well-being through a combination of non-surgical treatments including medication and injections; physical, occupational, and speech therapies; and the use of assistive devices.
Kristen has been a Registered Nurse for 12 years and a Nurse Practitioner for 7. She has experience in pediatrics, primary care, urgent care, and has also served as the Medical Director and director of a home health program for a ministry in Haiti where her family lived for several years. Kristen is married to a Missions Pastor in Jackson, TN and they share three children together: two boys ages 7 and 5, and a daughter who is 3. You can usually find Kristen chasing them around, driving them to sports games or church activities. Kristen and her family love to travel and explore new places together, and they continue to have a heart for overseas missions as well. Kristen states, “I am excited to be working at Christian Care Center of Medina! It is privilege to be able to serve these sweet patients and work with this wonderful staff!”
Wednesday was National Golf Day. Some of our Residents enjoyed a little Putt-Putt outside on our putting green.
Mr. Hopkins admitted to Christian Care Center of Medina with a decline in function limiting his independence in meaningful tasks.
He worked diligently with his Physical, Occupational and Speech Therapists to meet his goals and return home with his wife in less than a month! We are extremely proud of his hard work and dedication towards his rehabilitation success! Join us in congratulating Mr. Hopkins and wishing him well as he returns home!

We are thrilled to announce our new Rehab Director here at Christian Care Center of Medina, Brooklyn Morris!
Brooklyn is a fellow Gibson County Resident and loves to cheer for the SGC Hornets! She is married to Greg Morris, and they share a beautiful baby girl together, Charlie Elizabeth. She is graduate of Jackson State Community College and has served in the Long-Term Care/Skilled Nursing Sector for over 5 years.
Brooklyn strives to lead by example by providing exceptional and compassionate care to her patients. She is eager to serve her patients, their families, and her team!
