Meet Our Staff

LeeAnn Polasek 

LeeAnn brings 13 years of experience in long-term care as an Activities Director and is currently certified through Tennessee Healthcare Association. LeeAnn strives to make her residents feel at home and full of happiness.  

She currently resides in Humboldt and enjoys spending time with her children and grandchildren.

Tammy Bailey

Tammy brings 26 years of nursing experience specializing in Med Surge, acute care, orthopedics, and oncology. 

She enjoys camping, horseback riding and spending time with her grandchildren.

Sandi Ray

Sandi has over 18 years of Business Office Management experience in the nursing facility setting. She has an open door policy and team-oriented spirit and she is eager to assist in ensuring the best possible environment and care for our residents.

Sandi resides in Medina with her 19 year old daughter, Parris, and their three beloved dogs. She also has two adult children and five grandkids. When she isn't working, you will probably find her outside working in her yard.

Dr. Joe Fire

Dr. Fire has over 35 years of healthcare experience. He has a BS degree in Business Administration, MBA from Thomas Moore College and a PhD in Christian Counseling Psychology from Cornerstone University.

He is a licensed Pastoral Counselor, licensed Chaplain, ordained Minister and retired Psychologist. He resides in Jackson, TN with his wife of 38 years and enjoys spending time with his grandchildren, hiking, golfing and travelling to historical sites. 

Jackie Cobb 

Jackie has been certified with the Association of Nutrition and Foodservice (ANFP) since 2003.

She has 3 children and 5 grandchildren with one grandchild on the way. She loves spending time reading, traveling and spending time with her family.

Her goal in life is to be happy and be the best person she can be.

Jonah Hayes

Jonah is a 2014 Graduate from Tennessee College of Applied Technology where he earned a diploma in Industrial Maintenance. 

He has four years of experience as a Maintenance Director in the skilled nursing industry and is committed to providing a safe and comfortable environment for our residents. 

Jonah enjoys getting to know our residents and learning from them. Jonah’s hobbies include hunting, fishing, and kayaking. 

Catrina Quenon

Catrina comes to Christian Care Center of Medina with over 30 years of nursing experience. She received her CNA certification in 1992 and in 2000, she obtained her RN license. Catrina has over 15 years of experience in Nursing Management.

Catrina resides in Milan with her husband of 30 years and her son and daughter. She has 2 adult children and 2 grandchildren. In her spare time, she enjoys gardening, horseback riding, and hunting. 

Jennifer Bradfield 

Jennifer comes to us with over 20 years of experience in skilled and long-term care. She is originally from Colorado but has proudly called West TN her home for the past 20 years. 

She resides in Medina with her husband of 15 years. She is the proud mother of 1 son and 3 daughters. In her spare time, she enjoys crafting and spending time with her husband and kids.

Brad Rogers

Brad leads and coordinates the daily operations with a passion of creating a family atmosphere and the highest quality of care possible. 

He holds a A.A.S degree in Physical Therapy from Jackson State Community College and BS degree in Organizational Leadership with a focus in Healthcare Management from Union University.

As a native of Medina, Brad anhis wife Jennifer have three children. He serves as the Scout Master for Troop 48 in Medina and enjoys time with family and friends.

Misty West

Misty has a wide variety of experience in Human resources, payroll, accounting, and production/material handling.

She is a 2006 graduate from the University of Tennessee at Martin and holds a BA degree in Accounting. Misty lives in Carroll County where she enjoys being outdoors and is involved in pet rescue.

Amy Mangium

Amy has 32 years of experience in skilled nursing facilities as a CNA, LPN, Charge Nurse and RN. She also has experience as a MDS Coordinator, Director of Nursing, and an Assistant Director of Nursing. Amy is a graduate of Dyersburg State Community College.

Amy resides in Jackson, TN with her husband and daughter. She also has three adult children and one granddaughter. She enjoys spending time with family and friends, attending church and believes in servant leadership.

Donna Scott

Donna has been a nurse for 12 years with  experience in acute care, home health, long-term and skilled care as  well as experience in care coordination, staffing and as an ADON. 

Donna has been married to her husband for 22 years and has 4 children and one  grandson. In her spare time, Donna enjoys traveling, attending football  and baseball games, and spending time with family.
